Instructions for Completing the Application Form for Post Graduate (PG) Admission 2012-13. [ Ph.D. / M.Tech (ICT) / M.Sc. (IT) / M.Sc. (ICT in ARD) / M.Des. ] Candidates are advised to follow the following instructions carefully.

• Submit separate application forms and make separate payments if you wish to apply for multiple programs.
• Write your name in capital letters as given in your certificate of the or your first Board/University Examination.
• Provide the details of your qualifying degree. Specifically mention the degree and the discipline so that your eligibility can be ascertained. If your final examination results are not declared, provide the expected date of result and latest results.
• For M. Tech. (ICT)only. GATE qualification is mandatory for admission to M.Tech.(ICT) Program. Only a valid GATE score in Electronics and Communication (EC), Computer Science (CS), or Information Technology(IT) discipline would be considered. Provide all the details including Registration No., Year, Subject Code, Discipline, All India Rank and Gate Score. Please mention any three areas of research interest from the following: VLSI & Embedded Systems, Communication Systems, Computer Networks and Machine Intelligence.
• Application fee (non-refundable) for online submission is Rs. 1000/- while for submission by post or courier (offline) it is Rs. 1100/-. The application fee is to be paid through a Bank Demand Draft, drawn in favour of "DA-IICT" and payable at Ahmedabad.
• For Online Application submission: Candidates submitting online application form are required to send a bank demand draft of Rs. 1000/- at the following address. Candidate must write his/her name, application no. and the name of the program applied for on the reverse side of the draft.
• For Offline Application submission: Candidates submitting offline application form are required to send application form along with the bank demand draft of Rs.1100/- at the following address. Candidate must write his/her name, and the name of the program applied for on the reverse side of the draft.
• The last date for receiving the completed application is 07 May 2012.
